Consent to Receive Advertising Mailings and to the Processing of Personal Data

I hereby, acting freely, of my own will and in my own interest, and confirming my legal capacity, in accordance with Article 18 of Federal Law No. 38-FZ "On Advertising" dated March 13, 2006, and Articles 6 and 9 of Federal Law No. 152-FZ "On Personal Data" dated July 27, 2006 (as amended and supplemented), confirm that I wish to receive the information/mailings specified below, and give my consent (hereinafter, the "Consent") to LLC "PB" (OGRN 1257400032284, TIN (INN) 7448265179, 84 Komsomolsky Prospekt, apt. 62, Chelyabinsk, Chelyabinsk Region, 454014) (hereinafter, the "Administration") to:

  1. the processing of my personal data for the purpose of promoting the products and services of the Administration and its partners (here and throughout this Consent, "partners" means legal entities/individual entrepreneurs in respect of whose products and services the Administration may, among other things, send advertising messages via telecommunication networks under a relevant agreement with such partners) by sending informational, advertising and other offers via telecommunication networks by means of push notifications, email, messengers, social networks, and the "puzzlebot" program (website).

The list of personal data to the processing of which I give my Consent: an identifier generated by the Administration, my last name, first name, email address, the indication that Consent has been given, the date and time the Consent was given,

I warrant that my personal data is accurate, up to date and belongs to me. The personal data is not publicly available, and it does not constitute special categories of personal data or biometric personal data.

Methods of personal data processing: automated and non-automated processing.

The list of actions with personal data that the Administration is authorized to perform under this Consent: collection, receipt, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, depersonalization, blocking, deletion, destruction, and transfer (provision, access) of personal data to the Administration's partners that provide the Administration with services of sending advertising offers/information on the basis of this Consent, as well as the Administration's engagement of others to process (collect, receive, record, systematize, accumulate, store, clarify (update, modify), retrieve, use, depersonalize, block, delete, destroy) my above-mentioned personal data for the purposes of this Consent.

Detailed information on the processing of personal data, my rights as a personal data subject and the ways to exercise them is set out in the Personal Data Processing and Protection Policy and the Consent to the Processing of Personal Data, which I understand, agree with and have acceded to in full.

  1. sending me news, service updates and informational materials, invitations to events, notifications of promotions and new products, messages, advertising offers and any other information of an advertising nature (including in the form of advertising and informational mailings), marketing communications, notifications and materials about the products, services and offerings of the Administration and its partners (hereinafter, "Advertising") via telecommunication networks by means of:

I confirm and warrant that the email address and/or other personal data I have provided to the Administration belong to me, that the Advertising sent to them is intended for me only and will not be received by third parties who have not given the Administration prior consent to receive Advertising, and that if I stop using my email address, or the services of the "puzzlebot" website, I undertake to notify the Administration immediately.

By providing the Administration with a phone number and/or email address or other data belonging to a third party, I warrant that I have obtained that third party's consent to receive advertising mailings from the Administration in accordance with and to the extent of this Consent.

I undertake not to use the personal data of others and undertake to compensate the Administration for any damage and losses (including those arising from administrative or other liability imposed on the Administration) resulting from my use and provision of the personal data of others.

To achieve the purposes of processing, the Administration may engage technical support services, mailing services and other technical services that process personal data on its behalf and in accordance with the requirements of the legislation of the Russian Federation, and may entrust such third parties with the processing of the personal data provided. I hereby consent to the transfer of my personal data to, and its processing by, the above-mentioned third parties, including but not limited to:

Such third parties are entitled to process my personal data on the basis of this Consent. The Administration is entitled to choose such third parties without further approval.

In turn, the Administration warrants that it will not transfer personal data to persons who do not need it. The Administration also warrants that the transfer of personal data will be limited to the achievement of specific, predetermined and lawful purposes, and that the content and scope of the personal data transferred will correspond to the stated purposes of processing.

  1. The Consent is valid from the moment it is given until the mailings are discontinued or until the day it is withdrawn.

I confirm that I have been informed that I may at any time change my advertising mailing settings or completely opt out of receiving Advertising using the relevant section of my Personal Account (to access this section, I must be logged in to the "puzzlebot" website or the Administration's application).

The Consent may be withdrawn by sending a written notice to the Administration at 84 Komsomolsky Prospekt, apt. 62, Chelyabinsk, Chelyabinsk Region, 454014, or by clicking the special "Unsubscribe" link in the emails sent, or by sending a corresponding opt-out request to .

  1. The term of the Consent is until I withdraw it, since until such withdrawal the receipt of information/mailings/Advertising is deemed relevant.

Withdrawal of the Consent does not affect the lawfulness of the processing of personal data or of the receipt of information/mailings/Advertising carried out before its withdrawal.

If I or my representative withdraws the Consent, the Administration may continue processing without my permission where there are grounds specified in clauses 2 to 11 of Part 1 of Article 6, Part 2 of Article 10 and Part 2 of Article 11 of Federal Law No. 152-FZ "On Personal Data" dated July 27, 2006.

  1. I confirm that I have full legal capacity and may independently give Consent to receive Advertising and to the processing of my personal data for this purpose.

I give this Consent voluntarily, freely and in my own interest; it is specific, informed, conscious, unambiguous and comprehensive in its subject matter.

I understand that ticking the checkbox to the left of the phrase "I have read and agree to receive Advertising mailings" means that I have read and fully understood this text, have received clear and complete answers to my questions, and also constitutes my written voluntary consent to the provisions of this "Consent to Receive Advertising Mailings and to the Processing of Personal Data".
